Which Passport Application Do You Need?
New Passport (DS-11)
Use this option if any of the following apply:
– This is your first U.S. passport
– Your previous passport was issued when you were under 16
– Your previous passport was issued more than 15 years ago
– Your previous passport is damaged
– Your previous passport was lost or stolen and you have already reported it
– You are applying for a child under 16
Passport Renewal (DS-82)
Use this option if all of the following apply:
– Your current passport was issued less than 15 years ago
– You have the passport in your possession and it is undamaged
– You were 16 or older when it was issued
– It was issued in your current name, or you can document your name change
Lost or Stolen Passport (DS-11 + DS-64)
Use this option if:
– Your passport was lost or stolen and you have not yet reported it
– You need to report it and apply for a replacement at the same time

Which form will I need — DS-11, DS-82, or DS-64
That depends on your situation. DS-82 is the renewal application, available if your passport was issued in the last 15 years, when you were 16 or older, is undamaged and in your possession, and is in your current name. DS-11 covers first-time applicants, children under 16, damaged passports, and passports issued too long ago to renew. DS-64 reports a lost or stolen passport and is prepared alongside a DS-11.
